The affidavit documents a candidate’s successful completion of a performance assessment. 

As of July 1, 2019, evaluators submit affidavits online after being assigned to evaluate a candidate's performance by an account administrator. This includes both project performance types and CAR performance types.

Measurement, Materials, and Safety (MMS) does not include a performance assessment and does not require an affidavit.
